In order to shorten the waiting time at Aritake Eye Cline, we are
interoducing a reservation system. You can now make an appointment by
phone.
How the system works
Features of the telephone appointment system
Enter the numbers and symbols according to the directions.
You can use push-button phones and dial lines as well as mobile phones
and pay phones.
Your examination ticket number is required when making a reservation.
(First time patients are not required to use this.)
Please be ready to enter your medical registration card number
Reservation hours
24-Hour reception. You can make a reservation 30 days in advance.
Other considerations
Please note that if you arrive at the clinic without first making a
reservation, you may have to wait longer. Please be sure to cancel your
reservation if you are unable to attend.
First time patients may also make a telephone appointment.
How to use the phone appointment system
1. Reservation-only number: 0561-87-0489. Follow the voice guidance.
2. Press the "＊"mark button twice.
3. Enter your medical registration card number and then press"#". New
patients please press "#" twice. Then, enter your phone number.
4. Enter your date of birth and then press"#". For example: January 1, is
"0101#″(enter four digits). Do not enter your birth year.
5. Enter the service code and then press"#". For example: Examination
「11#」, Contact lens 「12#」, Super Laser「13#」, Confirmation of
appointment 「20#」, Appointment cancellation「30#」, Check
appointment availability (desired day and time)「40#」.
6 .You may confirm your service code by pressing 「0#」.
7. To make an appointment (enter four digits). For example: if First of
July「0701」and press#.
8. Enter booking time. For example: if at 8:45 am 「0845」 and press #. If
at 4:30 pm 「1630」 and press#.
9. You can check the date of appointment by pressing 0#. If you are
unable to make an appointment at the desired date/time, we will
suggest an alternative. If this isn't convenient, press 1# and try again.
Be sure to enter 0# after making your appointment.
Be sure to end your reservation by pressing 0#. Do not hang up after
hearing confirmation of your reservation time/date.
10. It is possible to make a reservation for up to 3 people. This can
include family members as well as friends. Press 1#, enter your
examination number, and press # again.
11. Please be at the clinic 5 minutes prior to the time of your appointment.
Please remember that medical emergencies take priority. Therefore, we
cannot guarantee that you will be seen at your appointed time.
However, we will do our utmost to ensure that your wait isn't too long.
Even if you are unable to make an appointment at your desired time/date,
please feel free to come to the clinic at anytime as we will be able to see you.
